What it does
Most post-mortems fail before they start. Too wide a target, the wrong people in the room, or a worry that someone will be blamed. Then the review gives a shallow answer or a scapegoat, and the problem comes back. This skill sets up the review. It narrows the target to one class of failure and writes a trigger rule that needs no judgment. It lists everyone who must attend, including the person you would rather leave out. It names a moderator, sets two simple rules and gives a line for when blame starts. It ends with an agenda with minutes.
