What it does
When a problem comes back, the usual fix patches the surface. A late order is blamed on a person, a bug on a rushed release, and the same thing happens next month. This skill asks why several times in a row, and each answer must come with evidence: a log line, a photo, a person who was there. If there is none, the answer is marked unconfirmed. It tries a second branch in case there is more than one cause and keeps the chain about the process, not the person. It ends with a fix that removes the cause and a check to prove the problem stays away.
