What it does
A blameless postmortem of an outage, written so your team learns from it and the same failure does not return. You give the facts and get a document with a timeline, causes and follow-up actions that have owners. The skill swaps blame for system questions. It asks what let the mistake happen, not who made it. It builds a minute-by-minute timeline, separates the root cause from contributing factors, and measures impact in users, money and time. Each fix becomes a numbered action with an owner and a due date. It also gives a way to run the review meeting so people speak freely, plus templates for short and long incidents.
You give it
- Incident notes, chat logs or alerts
- Times of detection, response and fix
- Impact figures (optional)
- Who was involved (optional)
